Share a Form

A public form has its own address, which anyone can open without signing in:

https://mychurchhq.com/f/your-church/form-name

Steps

1. Choose Forms in the sidebar and open the form.
2. Check it is set to Public, if you want anyone to be able to fill it in.
3. Copy its link, or print the QR code, and put it where people will see it: your website, a message, the screen, a card on the seats.

Where the answers go

Open the form and choose its submissions to read what has come in. Where an answer matches someone your church already knows, Church HQ shows the match so you can attach it to that person.

Good to know

• Church HQ never fills in a person’s email or phone from a public form on its own. A stranger could type anything; your team decides what to keep. That is deliberate.
• Internal forms live in the Member Portal instead, for people who are signed in. See Fill Out a Members-Only Form.
